if someone plays 4-4-2 should the one mc have defensive duties?
also if you play push up defensive line and counter attack what will hapen?

Yeah. You need someone that roles as a MCd and someone with roles MCa. MCd can be more defensive and he can connect from defense to attacking.
 
Think of the balance of the team. If you want an attacking central midfielder, you need a defensive midfielder. Or you could have two players who are somewhere in the middle of the two extremes. It's easier to divide the two roles then to find two perfectly balanced central midfielders.

Not sure what you mean by pushing up the d-line and ticking counterattack? If you play with all the settings required to successfully use a high D-line, then the chances are that your counterattacks will start higher up the pitch. Downside may be that you'll leave yourself wide open for a counter-counterattack if you lose possession though.
 
It depends on your philosophy, if u like attack, u usually only need 3-4 defenders, 3-5 supports, and 3-4 attacking. If you have a very athletic and good back four, you can utilize a more control/offensive orriented midfield, where u dont really need a DMC, but rather creative once that can help you control the tempo wihtout making critical mistakes. But if you go defensive, you should have at least 5-7 defensive mentality dudes with 3 support and probably 1-2 attacking. Or you can use standard pitch and play balance, where a good DMC will be useful to fill in the gap between your d-line and your midfield.
 
Usually i always have counter-attack ticked no matter how high i play up the pitch. I just find it useful.

An interesting method i found was using a DM with some skill like Palombo playing the AM role alongside a more defensive DM. Works a treat in keeping posession/preventing attacks. Especially with two wingers and 2 strikers bridging the gap between midfield and attack.
 
Clough used to play Roy Keane in that kind of role at Nottingham Forest, he paired him with a deeplying playmaker (Neil Webb IIRC). ;)
