When evaluating players, which one is more important, good form or good attributes? Is a player with slightly worse attributes but good form more valuable than a less in-form high attributes player?

Yep, form is just that. Form. Unlikely to last. Go by attributes, assistant report, gut feeling and then form.
 
Stats all day every day. But if the scout report mentions injury proneness, poor consistency or important matches I won't sign them.
